Rheumatoid arthritis (RA) is a chronic, inflammatory disorder primarily of the synovial joints, but can also affect other systemic organs. RA affects over 2 million Americans, with women being 3 times more susceptible than men.

The Arthritis Foundation states this disease progresses through three stages:

1) First is swelling of the synovial lining, causing pain, warmth, stiffness, redness and swelling around the joint.
2) Second is the rapid division and growth of cells, which causes the synovium to thicken.
3) In the third stage, the inflamed cells release enzymes that may digest bone and cartilage, often causing the involved joint to lose its shape and alignment, more pain, and loss of movement.

Eventually, if left untreated, RA has the capacity to destroy cartilage, deform joints, and destroy adjacent bones. And because it is a systemic disease it can affect the entire body. RA can cause generalized inflammation in the cardiac muscle, in blood vessels, and within layers of the skin. The exact cause of RA is unknown, but it is considered an autoimmune disorder (the body attacks itself). Genetic predispositions, or viral infections are thought to increase risks of RA.

People experiencing rheumatoid arthritis can be best helped the earlier it is caught, especially before excessive suppression has taken place from conventional methods of treatment. In homeopathy, health is viewed as a condition of the entire individual, rather than in terms of isolated symptoms from specific locations of the body. Rhus Toxicodendron: The main homeopathic remedy considered from rheumatism with pain and stiffness. The pain and stiffness causes the person to shift and stretch. Worse in the morning upon rising; in the cold, damp, rainy weather; storms; and sitting for long periods of time (such as in a movie or a car ride). They feel better from heat, hot bathing, or showers; and continued, gentle motion.

Arnica: Great soreness all over. Even the bed feels too hard. They feel bruised or beaten. Worse from being jarred or touched; they fear being touched. Pains are usually symmetrical.

Aurum Metallicum: Rheumatism with stiffness or spasms of the chest wall. This is an important remedy to consider for people who also have ankylosing spondylitis. Generally worse at night.

Ledum: Swelling, coldness and pallor (sometimes blueness) in the affected joint. Worse from heat. In general, the pains ascend during the course of the illness to more proximal joints.

Pulsatilla: Wandering arthritis; changeable symptoms and pains. Worse in the evening and with heat. Better from the cold; uncovering the joint; cold applications; and in the open air.

Homeopathic treatment will best assist people experiencing RA from constitutional, long-term care and, as mentioned, in the earlier stages of the illness. Even if medications have already begun, homeopathic treatment can help boost vitality and strengthen the healing intelligence of the body enough where the allopathic medications can eventually be discontinued altogether.

Liver is an important organ of the body. It performs several important functions in maintaining good health. These include production of proteins, detoxification of body, and storage of vitamin A. In addition to this, liver regulates supply of glucose and lipids to be used by the body. For the proper functioning, the liver cells and blood must work closely with each other. But in cirrhosis, the relationship between liver cells and blood is lost and it is unable to add or remove substances from the blood. In cirrhosis, the healthy liver cells are replaced by scar tissue and obstruct the normal flow of blood in the liver. As a result, blood starts accumulating in the portal veins producing the condition called portal hypertension where blood travels in other veins which bypass the liver to organs that performs the function. The relation between liver and canaliculi carrying bile also gets disturbed and liver is unable to perform the function of removing toxic substances from the body. It is this mechanism which leads to the symptoms of cirrhosis of liver.

There can be many causes of cirrhosis of liver.

• The most common cause can be chronic intake of alcohol in large quantities. It causes the death of healthy liver cells and they result in scarring around your veins of liver.
• Hepatitis that is the inflammation of liver due to viral infection can also cause serious damage to your liver cells and cause cirrhosis. Hepatitis B, hepatitis C and Hepatitis D are all culprits.
• Many genetic diseases can be involved in your liver cirrhosis. Diseases such as glycogen storage disease, hemochromatism, Wilsons disease, galactosemia and cystic fibrosis causes abnormal accumulation of toxic substances resulting in damage to liver cells.
• A condition called non-alcoholic fatty liver also causes cirrhosis of liver. It occurs due to certain diseases like CAD, obesity, type II diabetes and protein malnutrition disease.
• Other conditions which are known to cause liver cirrhosis are biliary cirrhosis, autoimmune cirrhosis, cardiac cirrhosis, long term bacterial or parasitic infection and many drugs.

At the initial stage of your cirrhosis of liver, you might not get any signs and symptoms. But with your progression of disease, you can experience weakness or tiredness, appetite loss, weight loss, paleness of skin with nausea, loss of libido. Sometimes you start getting symptoms when the complications of cirrhosis start taking toll. You can get itching due to deposition of breakdown of bile products. Due to liver enlargement, abdominal pain can become a normal routine. Sometimes, fluid retention leads to weight gain and swelling in abdomen, ankles and legs with breathing problems. Due to cirrhosis of liver, the toxins are not eliminated from body and affect your brain resulting in symptoms like delusions, hallucinations, confusion of mind, sleepiness and sometimes even comatose. As the stage progresses, you start getting bruises with slight injury or touch and gum bleeding. Liver congestion can also result in blood in your feces and vomiting. Exercise is very important because can help improve the blood flow in our system. When you’re a patient suffering from fibromyalgia, exercises that are high intensity or high impact is never a good idea. It can leave you in extreme pain for days after your workout.     I remember the night I woke up with kidney stones. A twinge ran down through my urethra and penis. It jolted me awake from a sound sleep. I felt a demanding sensation to urinate. However, not much would come out. Not knowing what the problem was I did immediately start drinking as much water as I could handle. Finally, after about an hour of freaking out, the two Extra Strength Tylenol I took knocked me out.

    Of course, at the time this happened I did not know that I had kidney stones. That took about a week or two later after a bunch of visits to various doctors, tests and finally an X-ray. Now 26 years later I still have bouts with these demon stones. Luckily, I only get stones now about once every 10 years or so compared to every three years.

    The pain and what I call “stone sign” has been different on each occasion. Just over 11 years ago, it was so bad I had to go to the emergency room where they gave me something like Demerol to ease the pain. On this occasion, the pain literally made me sick to my stomach. Two weeks later, I passed the stone, which did not hurt. It just felt odd.

    More recently, my latest kidney stone just made me uncomfortable enough to curl up into a ball on the floor and try to pass out for a couple hours. After that, the pain was gone but I had a reoccurring odd feeling when I urinated. This lasted for over a month before the stone finally passed. A stone had never lingered like this before and this was the largest stone I have passed so far at a quarter inch long.

    I have yet to find a doctor who conveys to me that they knew much about these stones or how to get them to stop. Their suggestions were suspect or too extreme for something that only happens to me once every 10 years or so. It turns out that not much research is definite on kidney stones. My stones are the common calcium based variety, which means my calcium to phosphorus balance is off. My body is running slightly more acidic than alkaline and that is where my problem lies. If your body runs acidic, you probably will create kidney stones.

    Since no doctors were very helpful, I decided to try a nutritionist. I found a nutritionist who claimed to have luck with several of her clients as well as some close friends. I am trying her suggestions and if you suffer from kidney stones, you might be interested in trying them too. She swears by them and since these guidelines were reasonable, I felt it worth a try for me.

    The main idea of the guidelines is to try to drink lots of water, keep acidic foods to a minimum, and take a few supplements to keep urine from clumping or increase a more alkaline state in the body. In addition, work on keeping stress down too. Exercise is essential to keep a balance and working that stress out.

    I must drink at least six glasses of water a day. Staying hydrated is critical for people who form stones. It is one of our main concerns. Drinks I try to avoid are most teas, soft drinks, alcohol, and coffee. These are all high in acid. It is okay to drink electrolyte drinks such as cranberry juice, Performance, peppermint ginger tea, and alfalfa tea.

    The idea is to stay away from acid forming foods and try for a 70% alkaline forming food base. The foods I try to keep to a minimum are red meat, spinach, parsley, asparagus, anything with sugar because this increases acid production, chocolate, rhubarb, chard, beet greens, dairy products, and especially cheese. The problem is many of these offer valuable nutrition and minerals you need. Therefore, I still eat these but I regulate my intake. Many of the vegetables when cooked create a less acidic state. Dairy products are high in calcium, which is important as well. Therefore, there is some give and take here.

    I now take various supplements to help keep the stones at bay and they are as follows:

    To keep urine more fluid I take Calcium Magnesium. This is also helps with keeping my calcium to phosphorus level balanced. Magnesium also works in helping keep calcium depletion at a minimum.

    I also take a B Complex supplement because this helps keep a balance of oxalate acid which tends to encourage stone formation. Oxalate acid can interfere with calcium absorption. When you have too much oxalate in your system, it may combine with calcium and this is a common method of kidney stone formation.

    I take some Vitamin C to aid in heading off infections. Occasionally stones are found to be associated with chronic infections. I do not have that condition but a little extra help in the form of Vitamin C seems like a good idea. On top of this, I drink a glass of water with half a lemon squeezed into it. This is supposed to help with breaking stones down and keeping your system alkaline based. It must be taken after you have chewed some food though to let the proper enzymes start working. Lemon water on its own can be acidic. However, if you drink it while eating, the enzymes keep it alkaline.

    Trying to keep my body more alkaline is a challenge. Many of the foods that are acidic I enjoy. To boost my alkaline level I take alfalfa tablets. You have to research alfalfa tablets because some are not good. I am currently taking alfalfa tablets made by Shaklee, which I feel are very good. I am sure there are others that are just as good. Do your research to know which are best.

    Exercise must be performed daily. This is so important and yet so many people have difficulty fitting it into their schedules. Exercise is a vital balancing factor. Have you ever thought about how you take care of your stress? If you have not, you should. One key to keeping your stress level properly balanced is getting out there and moving everything. This helps with internal organ work as well as muscle function. Do whatever you can to fit it in at least a little every day.

    As you can see, there is a difficult to uphold balance your body needs to hold to keep your kidneys functioning right if you have a tendency to get stones. Tipping the scales too much one side or the other can cause stones. However, if you have an inclination towards getting kidney stones, you need to try to compensate for that lack of balance by doing things to fix the problem. I personally never want to experience a stone so big it has be surgically removed.

    While I cannot guarantee the guidelines above will work for everyone, I am trying them myself. They seem as good as anything else I have heard and my nutritionist swears by them. If you have questions, always ask your doctor. It is better to get several opinions before making big changes in your routine. Everyone is different and your specific conditions could be unusual. Therefore, do check with a doctor first.


    Hemostasis is made possible by formation of a platelet plug AND a fibrin clot. Both are needed, and a clinician needs to think about them differently. Let’s explore platelet plug formation.

    Now, there are two main problems with platelets, either 1) a decreased number or 2) bad (broken) platelets that have poor function. There are two tests that we need to evaluate for platelet function.

    One is the platelet count as seen on the red blood cell count. So, we are looking at the absolute number, or we are looking at a test called the bleeding time. When a platelet count is decreased, this is known as thrombocytopenia. This is seen if there is a problem in the bone marrow such as decreased production of platelets which you can see in some malignancies, most commonly seen in chemotherapy that suppresses bone marrow, increased destruction such as a hyperactive spleen where the only potential solution to this is to have a splenectomy, or from idiopathic thrombocytopenia which is typically a child after a viral syndrome where the treatment is steroid removal immunoglobulin or spleen removal. When I was in my pediatric rotation, I remember seeing a young child for a well child visit, and this was actually my first rotation as a physician assistant student. When the physician came in, the child clearly responded adversely to the pediatrician who was a bit older with a beard, and the mom said that he looked kind of like his father. The child was really kind of frightful of the physician. When he looked over the child, the child had a number of bruises especially on his lower legs. I believed this child was being abused by his father. The doctor sent him off for blood work, and I was quite upset with the physician which I talked about wondering how he could not see how apparent abuse was. Well, when the blood work came back, this child had idiopathic thrombocytopenia, and it was not child abuse.

    Sepsis, in the setting of a low platelet count, is a red flag for a patient requiring intensive care. If the platelet count is less then 50K, they have an increased risk of post traumatic bleeding. If you have platelet counts of less than 20,000, you can have increased spontaneous bleeding. When I used to cover the hematology/ oncology floor, we would transfuse patients when they got below 10,000.

    The bleeding time is a test of platelet function. The bleeding time is basically a small incision in the skin that by spring-loaded apparatus, it gives us a very uniform little abrasion, and the lab will blot that area until the bleeding stops. I may have only ordered this test once in my life. It is pretty rare. The most common reason why platelets would not clot well is aspirin, someone who takes aspirin, their blood is a little bit thin. Or uremia. Hypothermia can do this as well.

    The lifespan of a platelet is 7 days. Aspirin deactivated the platelet for that length of time. We will exclude genetic coagulopathies here. But, I do want to talk about medications that affect the platelets. The most common one by far is aspirin. The dose can vary between 81 mg to 1,000 mg daily.
